Article L112-7 of the French Insurance Code

Where an insurance contract is offered under the freedom to provide services within the meaning of article L. 362-2 or by a branch pursuant to article L. 362-1, the policyholder is informed, before any commitment is entered into, of the name of the Member State of the European Union in which the head office of the company or, where applicable, the branch with which the contract will be concluded is located.

The information mentioned in the previous paragraph must appear on all documents given to the policyholder or insured.

The contract or the cover note must indicate the address of the head office of the insurance company granting the cover, or where applicable that of the branch, as well as the name and address of the representative referred to in article L. 362-3.

Article L112-7

Lorsqu’un contrat d’assurance est proposé en libre prestation de services au sens de l’article L. 362-2 ou par une succursale en application de l’article L. 362-1, le souscripteur est informé, avant la conclusion de tout engagement, du nom de l’Etat membre de l’Union européenne où est situé le siège social de l’entreprise ou, le cas échéant, la succursale avec laquelle le contrat sera conclu.

Les informations mentionnées à l’alinéa précédent doivent figurer sur tous documents remis au souscripteur ou à l’assuré.

Le contrat ou la note de couverture doit indiquer l’adresse du siège social de l’entreprise d’assurance qui accorde la couverture, ou le cas échéant celle de la succursale ainsi que le nom et l’adresse du représentant mentionné à l’article L. 362-3.
