Article L2112-5 of the French Public Health Code

The departmental mother and child protection service liaises with the school health service, in particular by forwarding to the school health doctor the medical records of children being monitored at nursery school. The way in which this is done must ensure that professional confidentiality is respected. These medical records are drawn up in accordance with a model laid down by interministerial order and sent before the medical examination carried out in application of article L. 2325-1.

Le service départemental de protection maternelle et infantile établit une liaison avec le service de santé scolaire, notamment en transmettant au médecin de santé scolaire les dossiers médicaux des enfants suivis à l’école maternelle. Les modalités de cette transmission doivent garantir le respect du secret professionnel. Ces dossiers médicaux sont établis conformément à un modèle fixé par arrêté interministériel et transmis avant l’examen médical pratiqué en application de l’article L. 2325-1.
