Article R1333-114 of the French Public Health Code

The registration application must be accompanied by a file containing :

1° In the case of a natural person, their surname, first name, position and contact details;

2° In the case of a legal entity, its name or corporate name, its legal form and the address of its registered office;

3° A description of the proposed nuclear activity;

4° Supporting evidence consisting of the information referred to in Article R. 1333-9;

5° Information about the establishment and in particular the precise location of the places where the proposed nuclear activity is to be carried out;

6° Information on the organisation of radiation protection, the sources of ionising radiation and the installation housing them;

7° The measures adopted to comply with the requirements of the general regulations and those applicable to the nuclear activity, in particular those of the decisions mentioned in Article R. 1333-113 of this code and Article L. 592-20 of the Environment Code.

Article R1333-114

La demande d’enregistrement est accompagnée d’un dossier comportant :

1° S’il s’agit d’une personne physique, ses nom, prénom, fonctions et coordonnées ;

2° S’il s’agit d’une personne morale, sa dénomination ou sa raison sociale, sa forme juridique et l’adresse de son siège social ;

3° Une description de l’activité nucléaire dont l’exercice est envisagé ;

4° Les éléments de justification constitués par les informations mentionnées à l’article R. 1333-9 ;

5° Des informations sur l’établissement et notamment la localisation précise des lieux d’exercice de l’activité nucléaire envisagée ;

6° Des informations sur l’organisation de la radioprotection, sur les sources de rayonnements ionisants et l’installation les abritant ;

7° Les dispositions retenues pour respecter les prescriptions de la réglementation générale et celles applicables à l’activité nucléaire, notamment celles des décisions mentionnées à l’article R. 1333-113 du présent code et à l’article L. 592-20 du code de l’environnement.
