I. – The certificate provided for in Article L. 2223-42 includes:
1° An administrative section comprising:
a) The commune of death;
b) The date and time of death;
c) The surname, first names, date of birth, sex and place of residence of the deceased;
d) The information required for the issue of the authorisation to close the coffin and for the performance of funeral operations ;
2° A medical section relating to the causes of death;
3° An additional medical section where medical or scientific research into the causes of death has been carried out under the conditions of article L. 1211-2 of the Public Health Code or a judicial autopsy has been ordered in accordance with article 230-28 of the code of criminal procedure. The information on this additional medical section confirms, supplements or replaces that on the medical section of the death certificate referred to in 2°.
Where applicable, the additional medical section is drawn up, as soon as possible, on an electronic medium, and sent to the same recipient as the medical section, under the conditions defined in III of Article R. 2213-1-2.
II. – The administrative section and the medical section are drawn up by the doctor, student or practitioner who ascertains and certifies the death.
The additional medical section is drawn up by the doctor who carries out the medical or scientific investigation into the causes of death or the forensic autopsy.
III. – The medical record and the additional medical record do not include the surname or forename of the deceased, or the registration number of the persons in the national register for the identification of natural persons.
