Each test includes :
1° Where applicable, informing the person in charge of the test of the athlete’s use of the health products defined in Article L. 5311-1 of the Public Health Code, in particular medicines, whether or not prescribed, or food supplements;
2° A medical examination carried out by the person responsible for the inspection, if he or she is a doctor and considers it necessary;
3° One or more of the samples and screening operations listed in article R. 232-50 of this Code;
4° The drafting and signing of the report.
The athlete may refer to a Therapeutic Use Exemption or an application for a Therapeutic Use Exemption as provided for in article L. 232-2 of the French Sports Code and provide any other information in support of his or her declarations. If the person in charge of the test is a doctor, he may also ask to see the individual medical and sports record book provided for in article L. 231-7 of the same code.
The athlete shall state on the report his date and place of birth, as well as his postal address and, if he has one, the e-mail address to which he will be sent the documents resulting from the test.
