When a blood sample is taken, the test report or an attached document, the model for which is determined by the College of the French Anti-Doping Agency, must include, in addition to the information provided for in article R. 232-58, the following information
– date and place of birth of the person concerned;
– nature of the sport practised;
– if the person concerned is a member of an approved national sports federation, the name of that federation;
– ambient temperature at the time of sampling;
– whether the athlete has remained in a seated position, with feet touching the ground, for a period of at least ten minutes prior to sampling;
– mention, if applicable, of the athlete’s participation in training courses at medium or high altitude, or the use of a device to simulate altitude, during the last two weeks;
– mention, during the same period, of a blood donation by the person concerned or of blood loss following medical problems or an emergency situation;
– mention, during the same period, of receiving a blood transfusion.
