The information referred to in article L. 4061-4 includes in particular:
1° Information relating to any disciplinary, professional or criminal sanctions imposed on the healthcare professional, as well as any appeals lodged by the latter against these decisions;
2° Information relating to the existence of a definitive discharge provided for in 4° of article L. 4139-14 of the Defence Code or a leave of absence as provided for in 1° or 2° of Article L. 4138-11 of the same Code;
3° Information relating to patient complaints against the healthcare professional or to serious and specific facts likely to be prejudicial to honour, probity or dignity or to have consequences for the professional’s professional practice;
4° Information on the skills and competencies acquired by the applicant during initial training, professional experience and lifelong learning that has been validated by a competent body.
