Article R4381-28 of the French Public Health Code

The application for registration of the company is submitted to the Director General of the Regional Health Agency of the company’s registered office, either collectively by the partners or by the partner designated as manager in the Articles of Association. It must be accompanied by

1° A copy of the Articles of Association and, if they have been drawn up, of the company’s internal rules and regulations and, where applicable, a copy of the instrument of incorporation;

2° For each partner :

a) a copy of the diploma, certificate or other evidence of formal qualification as a nurse or physiotherapist or proof of authorisation to practise the profession ;

b) An indication of the registration number of this diploma, certificate or title with the prefecture.

La demande d’inscription de la société est présentée au directeur général de l’agence régionale de santé du siège de la société soit collectivement par les associés, soit par l’associé désigné en qualité de gérant par les statuts. Elle est accompagnée :

1° D’un exemplaire des statuts et, s’il en a été établi, du règlement intérieur de la société ainsi que, le cas échéant, une expédition ou une copie de l’acte constitutif ;

2° Pour chaque associé :

a) D’une copie du diplôme, certificat ou autre titre d’infirmier ou de masseur-kinésithérapeute ou la justification de l’autorisation d’exercer la profession ;

b) De l’indication du numéro d’enregistrement à la préfecture de ce diplôme, certificat ou titre.
