Article R561-9 of the French Code governing the entry and residence of foreign nationals and the right of asylum

When the application is made on behalf of a minor, the applicant must submit the following documents in support of his or her application:
1° A document certifying the minor’s parentage;
2° A document certifying his or her status as legal representative;
3° Two recent and perfectly resembling 3.5 cm × 4.5 cm head-on photographs of the minor;
4° Proof of address or a declaration of domicile as referred to in article R. 551-8;
5° Where applicable, any document or evidence justifying that the minor is under the protection of the French Office for the Protection of Refugees and Stateless Persons.

Lorsque la demande est faite pour un mineur, le demandeur présente les pièces suivantes à l’appui de sa demande :
1° Un document attestant la filiation du mineur ;
2° Un document attestant sa qualité de représentant légal ;
3° Deux photographies de face du mineur, tête nue, de format 3,5 cm × 4,5 cm, récentes et parfaitement ressemblantes ;
4° Un justificatif de domicile ou une déclaration de domiciliation mentionnée à l’article R. 551-8 ;
5° Le cas échéant, tout document ou élément justifiant que le mineur est placé sous la protection de l’Office français de protection des réfugiés et apatrides.
