Article R612-10 of the French Intellectual Property Code

The application for grant shall be signed by the applicant or his representative. It shall contain:

1° The nature of the industrial property title applied for;

2° The title of the invention showing clearly and concisely the technical designation of the invention and containing no fancy names;

3° The designation of the inventor : however, if the applicant is not the inventor or the sole inventor, the designation shall be made in a separate document containing the surname, forenames and domicile of the inventor and the signature of the applicant or his agent;

4° The surname and forenames of the applicant, his nationality, domicile or registered office;

5° The name and address of the agent, if one has been appointed.

La requête en délivrance est signée du demandeur ou de son mandataire. Y figurent :

1° La nature du titre de propriété industrielle demandé ;

2° Le titre de l’invention faisant apparaître de manière claire et concise la désignation technique de l’invention et ne comportant aucune dénomination de fantaisie ;

3° La désignation de l’inventeur : toutefois, si le demandeur n’est pas l’inventeur ou l’unique inventeur, la désignation est effectuée dans un document séparé contenant les nom, prénoms et domicile de l’inventeur ainsi que la signature du demandeur ou de son mandataire ;

4° Les nom et prénoms du demandeur, sa nationalité, son domicile ou son siège ;

5° Le nom et l’adresse du mandataire, s’il en est constitué.
