I.-The authorisation holder provides care for the patient as part of a personalised care pathway. This care pathway provides for care adapted to the patient’s needs at the various stages of the pathway, integrating the gradation of care.

Where appropriate, it offers patients and their families therapeutic education programmes or actions.

II – Where necessary in complex situations, the authorisation holder organises multidisciplinary consultation meetings to discuss the care plan for the patients concerned.

I.-Le titulaire de l’autorisation prend en charge le patient dans le cadre d’un parcours de soins personnalisé. Ce parcours de soins prévoit une prise en charge adaptée aux besoins du patient aux différentes étapes du parcours intégrant la gradation des soins.

Le cas échéant, il propose au patient et à son entourage des programmes ou des actions d’éducation thérapeutique.

II.-Le titulaire de l’autorisation organise, en cas de besoin lié à des situations complexes, des réunions de concertation pluridisciplinaires traitant du projet de soins des patients concernés.
