Article R622-8 of the French Consumer Code

The document initiating proceedings shall contain, on pain of nullity, in addition to the particulars required by law, the name of the approved national association, its registered office, the body which legally represents it and the surname, forenames and address of each of the consumers on whose behalf it is acting.
Copy of the approval order issued pursuant to the provisions of article R. 811-2 is attached to the document initiating proceedings.

L’acte introductif d’instance contient, à peine de nullité, outre les mentions prévues par la loi, la dénomination de l’association nationale agréée, son siège social, l’organe qui la représente légalement et les nom, prénoms et adresse de chacun des consommateurs pour le compte desquels elle agit.
Copie de l’arrêté d’agrément pris en application des dispositions de l’article R. 811-2 est jointe à l’acte introductif d’instance.
