Article R623-12 of the French Consumer Code

The individual consumer information measures referred to in article L. 623-15 to which the professional must proceed must include, in addition to any particulars prescribed by the judgment:
1° A reproduction of the operative part of the judgment;
2° Details of the professional from whom each consumer may accept compensation and of the association that must be informed;
3° The form, content and deadline for acceptance of compensation under the terms of the judgment as well as an indication that this constitutes a mandate for the purposes of compensation in favour of the association to which the consumer has expressed his acceptance or which he has informed of this or, in the event of default, in favour of the association that will have been substituted for him;
4° An indication that a consumer who has accepted compensation under the terms of the judgment will no longer be able to bring an individual action against the trader concerned for compensation for the loss compensated under the group action but that he will still be able to bring an action for compensation for his other losses;
5° An indication that in the absence of acceptance under the required terms and timeframe the consumer will no longer be eligible to obtain compensation under the group action.

Article R623-12

Les mesures d’information individuelle des consommateurs mentionnées à l’article L. 623-15 auxquelles le professionnel doit procéder doivent comporter, outre les mentions éventuellement prescrites par le jugement :
1° La reproduction du dispositif de la décision ;
2° Les coordonnées du professionnel auprès duquel chaque consommateur peut accepter l’indemnisation et de l’association qui doit en être informée ;
3° La forme, le contenu et le délai de l’acceptation de l’indemnisation dans les termes du jugement ainsi que l’indication que celle-ci vaut mandat aux fins d’indemnisation au profit de l’association auprès de laquelle le consommateur a manifesté son acceptation ou qu’il a informée de celle-ci ou, en cas de défaillance, au profit de l’association qui lui aura été substituée ;
4° L’indication que le consommateur qui a accepté l’indemnisation dans les termes du jugement ne pourra plus agir individuellement à l’encontre du professionnel concerné en réparation du préjudice indemnisé dans le cadre de l’action de groupe mais qu’il pourra toujours agir en indemnisation de ses autres préjudices ;
5° L’indication qu’à défaut d’acceptation selon les modalités et délai requis le consommateur ne sera plus recevable à obtenir une indemnisation dans le cadre de l’action de groupe.
