Article R623-17 of the French Consumer Code

Subscription to the group shall be made by any means enabling receipt to be acknowledged, in accordance with the procedures determined by the judge.
In particular, it shall contain the consumer’s surname, first names, place of residence and, where appropriate, an e-mail address at which the consumer agrees to receive information relating to the proceedings.
It shall specify the amount claimed in compensation for the loss claimed, having regard to the provisions of the judgment on liability.

Article R623-17

L’adhésion au groupe est faite par tout moyen permettant d’en accuser la réception, selon les modalités déterminées par le juge.
Elle contient notamment les nom, prénoms, domicile du consommateur ainsi que, le cas échéant, une adresse électronique à laquelle celui-ci accepte de recevoir les informations relatives à la procédure.
Elle précise le montant demandé en réparation du préjudice invoqué, eu égard aux dispositions du jugement sur la responsabilité.
