A copy of the form required to renew the declaration provided for in article R. 212-88 is given in appendix II-12-2-b. This form specifies the list of documents required to renew the declaration.

When renewing the declaration, it is the responsibility of the Prefect of the département to ensure that the declarant has not been convicted of a crime or one of the offences mentioned in article L. 212-9 by requesting an extract from the criminal record (bulletin no. 2) dated less than three months from the judicial services.

Un exemplaire du formulaire nécessaire au renouvellement de la déclaration prévue à l’article R. 212-88 figure en annexe II-12-2-b. Ce formulaire précise la liste des pièces nécessaires au renouvellement de cette déclaration.

Lors de ce renouvellement, il appartient au préfet de département de s’assurer que le déclarant n’a pas fait l’objet d’une condamnation pour crime ou pour l’un des délits mentionnés à l’article L. 212-9 en demandant aux services judiciaires un extrait du casier judiciaire (bulletin n° 2) datant de moins de trois mois.
