At the time of renewal of the declaration, any person wishing to continue to exercise the functions covered by article L. 212-1 must produce a certificate of no contraindication to the practice and supervision of physical or sporting activities dating from less than one year before the day on which the application was submitted and is exempt from producing the supporting documents listed in the second paragraph of article A….

A standard declaration form for persons wishing to teach, supervise or lead physical or sporting activities, or to train those taking part in them, in return for remuneration is given in Annex II-12 to this Code. The subscriber attests on his or her honour to the accuracy and sincerity of the information given on the form.

Persons who have declared their activity in accordance with the provisions of the Order of 12 January 1994 retain the benefit of this declaration for the three-year period initially provided for in article R. 212-86. At the end of this period, they shall renew their declaration in accordance with the procedures set out in this code for a period of five years.
