The declaration provided for in articles R. 212-85 and R. 212-87, a specimen of which is given in appendix II-12, includes the surname, forenames, date and place of birth, nationality, contact details and place of residence of the persons concerned. For people born abroad, it also includes the surnames and first names of the father and mother. It mentions the diplomas, professional qualifications and professional qualification certificates, the date and place where they were obtained and the date of the last revision if the qualification is subject to revision. Where applicable, it shall mention the authorisation to practise, the diploma equivalence or, for persons undergoing training, the qualification being prepared, the practical training courses and the dates of the training booklet.
The declaration must be accompanied by a copy of a valid identity document, a passport photograph complying with the specifications of standard ISO/IEC 19794-5:2005, a declaration on honour attesting to the accuracy of the information given on the declaration form and a copy of each of the diplomas, qualifications and certificates referred to. Where applicable, a copy of the authorisation to practise, the diploma equivalence or, for trainees, the certificate attesting to the minimum requirements prior to being placed in a teaching situation and any document attesting to tutorship must be attached.
