The declaration provided for in article D. 322-13 shall be drawn up in triplicate. It shall include the surname, first name, date and place of birth and place of residence of the persons concerned, as well as their qualifications and diplomas.
It must be accompanied by a civil status form less than three months old, a copy of each of the qualifications cited and a medical certificate less than three months old stating that the person concerned has no apparent contraindication to swimming and lifesaving, or to supervising users of the establishments referred to in article D. 322-12.
This medical certificate, the model for which is set out in appendix III-9 to this code, must be renewed every year. If it is not renewed, the person concerned may not carry out the duties mentioned in article D. 322-13.
