The employee identification declaration referred to in article D. 133-13-1 of the Social Security Code includes the following information:
1° Information relating to the employee provided for in 2° and 3° of article R. 1221-1, relating to the declaration prior to recruitment;
2° Information relating to the job:
a) The nature of the employment contract: open-ended contract or fixed-term contract, with, in the latter case, an indication of the reason for use and the date on which the contract ends;
b) The duration of the work ;
c) The length of the trial period;
d) The job category, the nature of the job and, where applicable, the job level (hierarchical level and coefficient);
e) The title of the applicable collective agreement, if any;
f) The amount of remuneration and its various components, including, if applicable, bonuses and additional remuneration;
g) The particulars of the employment contract, if any;
h) The rate of contribution due in respect of accidents at work and occupational illnesses if several rates are applicable in the establishment;
i) Any deductions from the contribution base or rate;
j) The contribution rate for provident insurance, if it is specific to the employee;
k) Whether the employee is subject to transport contributions, if applicable;
l) Indication, where applicable, of a first recruitment in the establishment;
3° Signature of employer and employee.
A copy of the employee identification form is sent by the employer to the employee within the time limits specified in this code.
