Article D1272-5 of the French Labour Code

The employee identification form provided for in 2° of article D. 1272-1 contains the following information:
1° Information relating to the employee:
a) All the information provided for in article R. 1221-1, relating to the declaration prior to recruitment;
b) The employee’s affiliation to the general scheme or the agricultural scheme;
2° Information relating to the employment:
a) The end date of the job if it is a fixed-term job;
b) The length of the trial period;
c) The expected salary on recruitment;
d) The working hours;
e) The nature and category of the job;
f) The applicable collective agreement;
g) The rate of industrial accident contributions and, where applicable, the provident fund rate;
3° The signatures of the employer and the employee.

Le volet d’identification du salarié prévu au 2° de l’article D. 1272-1 comporte les mentions suivantes :
1° Mentions relatives au salarié :
a) L’ensemble des mentions prévues à l’article R. 1221-1, relatif à la déclaration préalable à l’embauche ;
b) Le régime d’affiliation du salarié au régime général ou au régime agricole ;
2° Mentions relatives à l’emploi :
a) La date de fin d’emploi s’il s’agit d’un emploi à durée déterminée ;
b) La durée de la période d’essai ;
c) Le salaire prévu à l’embauche ;
d) La durée du travail ;
e) La nature et la catégorie d’emploi ;
f) La convention collective applicable ;
g) Le taux de cotisations accidents du travail et, le cas échéant, le taux prévoyance ;
3° Les signatures de l’employeur et du salarié.
