Article D4625-28 of the French Labour Code

When joining, the employer shall provide the local occupational health and safety service with the following information:

1° The list of workers concerned, including those covered by enhanced individual monitoring ;

2° The address of the site or sites to be monitored;

3° The company sheet provided for in Article R. 4624-37;

4° The contact details of the main occupational health and prevention service, the occupational physicians and the health professionals mentioned in the first paragraph ofArticle L. 4624-1 who are competent.

Lors de son adhésion, l’employeur communique au service de prévention et de santé au travail de proximité les informations suivantes :

1° La liste des travailleurs concernés, dont ceux relevant d’un suivi individuel renforcé ;

2° L’adresse du site ou des sites à suivre ;

3° La fiche d’entreprise prévue à l’article R. 4624-37 ;

4° Les coordonnées du service de prévention et de santé au travail principal, des médecins du travail et des professionnels de santé mentionnés au premier alinéa de l’article L. 4624-1 compétents.
