Article D8223-1 of the French Labour Code

In application of article L. 8223-2, the employee obtains the information relating to the employer’s completion of the pre-recruitment declaration concerning him upon written request.
The employee’s request shall contain:
1° His or her surname, first names, nationality, date and place of birth;
2° His or her national identification number, if he or she is already registered with the social security system;
3° His or her address;
4° His or her date of recruitment and the period of work for which the information relating to the completion of the pre-recruitment declaration is requested.

En application de l’article L. 8223-2, le salarié obtient les informations relatives à l’accomplissement par l’employeur de la déclaration préalable à l’embauche le concernant sur demande écrite.
La demande du salarié contient :
1° Ses nom, prénoms, nationalité, date et lieu de naissance ;
2° Son numéro national d’identification, s’il est déjà immatriculé à la sécurité sociale ;
3° Son adresse ;
4° Sa date d’embauche et la période de travail pour laquelle l’information relative à l’accomplissement de la déclaration préalable à l’embauche est sollicitée.
