Article R1225-14 of the French Labour Code

At least fifteen days before the start of the parental presence leave, the employee shall inform his employer by registered letter with acknowledgement of receipt or delivered against receipt of his wish to take this leave. The employee must enclose a medical certificate.

When the parental presence leave is requested under the conditions provided for in the last paragraph of article L. 1225-62, he must also enclose the favourable opinion issued by the medical control department provided for in article L. 544-3 of the Social Security Code.

Au moins quinze jours avant le début du congé de présence parentale, le salarié informe son employeur par lettre recommandée avec avis de réception ou remise contre récépissé de sa volonté de bénéficier de ce congé. Il joint un certificat médical.

Lorsque le congé de présence parentale est demandé dans les conditions prévues au dernier alinéa de l’article L. 1225-62, il joint également l’avis favorable rendu par le service du contrôle médical prévu à l’article L. 544-3 du code de la sécurité sociale.
