The application submitted by an association shall include in particular:
1° A copy of the Journal officiel publishing the declaration of the association or, as regards the departments of Bas-Rhin, Haut-Rhin or Moselle, a copy of the extract from the register of associations of the judicial court;
2° A copy of the articles of association and, where applicable, the internal rules;
3° A list of the association’s establishments with an indication of their registered offices;
4° A statement indicating the operating conditions of the association and, where applicable, the organisation and operating conditions of the local committees and their relationship with the association ;
5° A statement of the surnames, first names, date and place of birth, nationality, profession and domicile of the members of the association’s board of directors and executive committee and, where applicable, those of its local representatives;
6° Financial documents, which must include the accounts for the last financial year, the budget for the current financial year and a balance sheet or statement of movable and immovable assets and liabilities ;
7° A list of the natural persons who, within the association, must carry out the tasks entrusted to it, with a mention of the surname, first names, date and place of birth, nationality, profession and domicile of each of them.
