The application for registration in the register referred to in a. of article L. 141-3 must be sent in writing, by electronic means if necessary, to the registration commission referred to in article L. 141-2.
The application for registration must be accompanied by documentary evidence of the financial guarantee and professional indemnity insurance defined in articles R. 211-26 to R. 211-40 , as well as proof of professional competence in application of article R. 211-41.
When the provisions of article R. 211-50 are applied, the application must be accompanied by documents proving that the applicant fulfils the conditions of activity laid down by this article.
When the application for registration is made by a natural person, it must state the applicant’s marital status, profession and place of residence, as well as the address of the registered office of the applicant’s activities and secondary establishments.
When the application for registration is made on behalf of a legal entity, it must state the company name, legal form, where applicable the amount of share capital, the address of the registered office and secondary establishments, as well as the marital status and domicile of the legal or statutory representative(s), the only person authorised to make the application.
When the application for registration is submitted on behalf of a federation or union of associations, it shall mention the name and address of the registered office of the associations or non-profit organisations that are members and for which it assumes responsibility.
