Article R232-67-4 of the French Sports Code

When a blood sample is taken, the test report or an attached document, the model for which is determined by the College of the French Anti-Doping Agency, must include, in addition to the information provided for in article R. 232-58, the following information

– date and place of birth of the person concerned;

– nature of the sport practised;

– if the person concerned is a member of an approved national sports federation, the name of that federation;

– ambient temperature at the time of sampling;

– whether the athlete has remained in a seated position, with feet touching the ground, for a period of at least ten minutes prior to sampling;

– mention, if applicable, of the athlete’s participation in training courses at medium or high altitude, or the use of a device to simulate altitude, during the last two weeks;

– mention, during the same period, of a blood donation by the person concerned or of blood loss following medical problems or an emergency situation;

– mention, during the same period, of receiving a blood transfusion.

Article R232-67-4

Lorsqu’il est procédé à un prélèvement sanguin, le procès-verbal de contrôle ou un document annexe, dont le modèle est arrêté par le collège de l’Agence française de lutte contre le dopage, doit comporter, outre les mentions prévues à l’article R. 232-58, les informations suivantes :

– date et lieu de naissance de l’intéressé ;

– nature du sport pratiqué ;

– en cas d’affiliation de l’intéressé à une fédération sportive nationale agréée, dénomination de cette dernière ;

– température ambiante au moment du prélèvement ;

– constat que le sportif est resté en position assise, ses pieds touchant le sol, pendant une période d’au moins dix minutes avant le prélèvement ;

– mention, s’il y a lieu, de sa participation à des stages en moyenne ou haute altitude, ou du recours à un dispositif de simulation de l’altitude, au cours des deux dernières semaines ;

– mention, au cours de la même période, d’un don du sang de la part de l’intéressé ou d’une perte de sang consécutive à des troubles médicaux ou à une situation d’urgence ;

– mention, au cours de la même période, du bénéfice d’une transfusion sanguine.
