Within ten days of the date on which the establishment opens, the director of the children’s home shall send the director general of the regional health agency a declaration accompanied by the medical certificates referred to in article R. 2321-25, containing :
1° The surname, forenames, title and place of residence of the doctor responsible for the medical supervision of the establishment, subject to approval by the Director General of the Regional Health Agency;
2° The surnames, forenames, dates of birth and titles of the management staff;
3° The surnames, first names and titles of the nurses and, where applicable, laboratory technicians and dieticians;
4° The names, with an indication of their position, of persons other than those previously designated, residing in the establishment in any capacity whatsoever, or working there without residing there.
Any person who is to be employed in the establishment and who was not included in the initial declaration must be the subject of a declaration made in accordance with the above provisions within ten days of taking up their duties.
