Article R3252-22 of the French Labour Code

The writ of attachment drawn up by the registry shall contain :
1° The surname, first names and domicile of the debtor and creditor or, in the case of a legal entity, its name and registered office;
2° A separate statement of the sums to be seized, including principal, costs and accrued interest, together with an indication of the interest rate;
3° The method of calculating the seizable fraction and the terms of payment;
4° An injunction to file the declaration provided for in article L. 3252-9;
5° The reproduction of articles L. 3252-9 and L. 3252-10.

Article R3252-22

L’acte de saisie établi par le greffe contient :
1° Les nom, prénoms et domicile du débiteur et du créancier ou, s’il s’agit d’une personne morale, sa dénomination et son siège social ;
2° Le décompte distinct des sommes pour lesquelles la saisie est pratiquée, en principal, frais et intérêts échus ainsi que l’indication du taux des intérêts ;
3° Le mode de calcul de la fraction saisissable et les modalités de son règlement ;
4° L’injonction d’effectuer au greffe, dans un délai de quinze jours, la déclaration prévue par l’article L. 3252-9 ;
5° La reproduction des articles L. 3252-9 et L. 3252-10.
