In the event of transferring his professional residence outside the department, the practitioner must apply, by registered letter with acknowledgement of receipt, to be struck off the register of the department where he was practising.
When the practitioner requests to be entered on the roll of the Association of his new professional residence, the Council of the Association of this department will take a decision under the conditions laid down in article R. 4112-2 and within a period of three months from receipt of the request, which may be extended by the departmental council for a period not exceeding two months where an expert opinion has been ordered.
A practitioner who ceases to practise in France must apply to the departmental council to be struck off the roll. This will take effect on the date on which the practitioner ceases to practise or, in the absence of any indication, on the date on which the request is received.
Decisions to strike doctors off the register are notified without delay in accordance with the conditions laid down in article R. 4112-4.
