Article R4113-126 of the French Public Health Code

The information sent, pursuant to Article L. 4113-1-1, by the bodies issuing evidence of formal qualifications to the service or body responsible for registering the persons referred to in Article L. 4113-1 are :

1° The civil status details of the holder of the evidence of formal qualifications or of the student undergoing training and any other identification details enabling the department or body responsible for registration to verify the identity of the applicant;

2° The name and address of the establishment or body that provided the training corresponding to the evidence of formal qualifications issued or the level of training certified;

3° The title of the evidence of formal qualifications, according to the classification provided for by the regulatory texts in force;

4° The level of training attained by students likely to be authorised to practise medicine, dentistry or the profession of midwife, under the conditions defined by articles L. 4131-2, L. 4141-4 or L. 4151-6.

Les informations transmises, en application de l’article L. 4113-1-1, par les organismes délivrant les titres de formation au service ou à l’organisme chargé de l’enregistrement des personnes mentionnées à l’article L. 4113-1 sont :

1° Les données d’état civil du titulaire du titre de formation ou de l’étudiant en cours de formation et les autres données d’identification permettant au service ou à l’organisme chargé de l’enregistrement de s’assurer de l’identité du demandeur ;

2° Le libellé et l’adresse de l’établissement ou de l’organisme ayant dispensé la formation correspondant au titre délivré ou au niveau de formation certifié ;

3° L’intitulé du titre de formation, selon la classification prévue par les textes réglementaires en vigueur ;

4° Le niveau de formation atteint par les étudiants susceptibles d’être autorisés à exercer la médecine, l’art dentaire ou la profession de sage-femme, dans les conditions définies par les articles L. 4131-2, L. 4141-4 ou L. 4151-6.
