In addition to the information mentioned in article L. 112-4, the contract includes

1° The surname, first names and date of birth of the insured person(s) ;

2° The event or date on which the guaranteed capital or annuity falls due;

3° The deadlines and terms of payment of the guaranteed capital or annuity.

Article R441-2

Le contrat comporte, outre les énonciations mentionnées à l’article L. 112-4 :

1° Les nom, prénoms et date de naissance du ou des assuré (s) ;

2° L’événement ou le terme duquel dépend l’exigibilité du capital ou de la rente garantis ;

3° Les délais et les modalités de règlement du capital ou de la rente garantis.
