Article R623-16 of the French Consumer Code

Information measures ordered pursuant to the provisions of article L. 623-7 include, in addition to any other information required by the judgment:
1° A reproduction of the operative part of the decision;
2° The contact details of the person with whom each consumer has indicated that he or she wishes to join the group and, where applicable, of the association that is to be informed;
3° The form, content and time limit of this membership as well as an indication that this membership constitutes a mandate for the purposes of compensation in favour of the applicant association with which the consumer has indicated that he or she wishes to join the group or which he or she has informed of his or her membership, or, if this association fails to act, in favour of the association that has been substituted for it;
4° An indication that, in the absence of membership received in the manner and within the period provided for by the judgment, the defaulting consumer will no longer be eligible to obtain compensation under the group action;
5° An indication that the consumer will no longer be able to act individually against the professional concerned for compensation for the loss already compensated under the group action but that he will still be able to act for compensation for his other losses;
6° An indication that consumers must produce any useful document in support of their claim.

Article R623-16

Les mesures d’information ordonnées en application des dispositions de l’article L. 623-7 comportent, outre les mentions éventuellement prescrites par le jugement :
1° La reproduction du dispositif de la décision ;
2° Les coordonnées de la personne auprès de laquelle chaque consommateur manifeste son adhésion au groupe et éventuellement de l’association qui doit en être informée ;
3° La forme, le contenu et le délai de cette adhésion ainsi que l’indication que celle-ci vaut mandat aux fins d’indemnisation au profit de l’association requérante auprès de laquelle le consommateur a manifesté son adhésion au groupe ou qu’il a informée de son adhésion, ou, en cas de défaillance de celle-ci, au profit de celle qui lui aura été substituée ;
4° L’indication que, à défaut d’adhésion reçue selon les modalités et dans le délai prévus par le jugement, le consommateur défaillant ne sera plus recevable à obtenir une indemnisation dans le cadre de l’action de groupe ;
5° L’indication que le consommateur ne pourra plus agir individuellement à l’encontre du professionnel concerné en réparation du préjudice déjà indemnisé dans le cadre de l’action de groupe mais qu’il pourra toujours agir en indemnisation de ses autres préjudices ;
6° L’indication que les consommateurs doivent produire tout document utile au soutien de leur demande.
