The holder of the authorisation provides assistance to primary care professionals, other health establishments and the medico-social establishments and services concerned in organising the assessment and evaluation of the patient, drawing up the patient’s therapeutic plan and facilitating referral.

Within this framework, the holder may set up telehealth activities and mobile teams.

Le titulaire de l’autorisation apporte son concours aux professionnels du premier recours, aux autres établissements de santé ainsi qu’aux établissements et services médico-sociaux concernés pour organiser le bilan et l’évaluation du patient, construire le projet thérapeutique de ce dernier et faciliter son orientation.

Dans ce cadre, le titulaire peut mettre en place des activités de télésanté et des équipes mobiles.
