Article D321-4 of the French Sports Code

The policies mentioned in article D. 321-1 are taken out on production of a certificate, in particular to the officials of the Ministry of Sports authorised in application of article L. 111-3.

This document is deemed to be a guarantee. It must include the following information:

1° The reference to the legal and regulatory provisions;

2° The name of the approved insurance company or companies;

3° The number of the insurance contract taken out;

4° The period of validity of the contract;

5° The name and address of the policyholder;

6° The scope and amount of cover.

Article D321-4

La souscription des contrats mentionnés à l’article D. 321-1 est justifiée par la production d’une attestation, notamment aux fonctionnaires du ministère chargé des sports habilités en application de l’article L. 111-3.

Ce document vaut présomption de garantie. Il comporte nécessairement les mentions suivantes :

1° La référence aux dispositions légales et réglementaires ;

2° La raison sociale de ou des entreprises d’assurances agréées ;

3° Le numéro du contrat d’assurance souscrit ;

4° La période de validité du contrat ;

5° Le nom et l’adresse du souscripteur ;

6° L’étendue et le montant des garanties.
