Article D4626-6 of the French Labour Code

Each year, the head of the establishment draws up a report on the organisation, operation and financial management of the independent occupational health and prevention service. This report is drawn up in accordance with a model laid down by joint order of the ministers responsible for health and labour.

When the autonomous occupational health and prevention service groups together several establishments under an agreement, a joint report is drawn up by the head of the establishment hosting the service. It describes the activities of the autonomous occupational health and prevention service in each of the establishments concerned. A copy of this report is sent to each establishment party to the agreement.

Le chef d’établissement établit chaque année un rapport sur l’organisation, le fonctionnement et la gestion financière du service autonome de prévention et de santé au travail. Il est élaboré selon un modèle fixé par arrêté conjoint des ministres chargés de la santé et du travail.

Lorsque le service autonome de prévention et de santé au travail regroupe par convention plusieurs établissements, un rapport commun est établi par le chef d’établissement hébergeant le service. Il retrace l’activité du service autonome de prévention et de santé au travail dans chacun des établissements concernés. Un exemplaire de ce rapport est adressé à chaque établissement partie à la convention.
