Article R1434-6 of the French Public Health Code

The regional health plan includes objectives aimed at :

1° Develop prevention and health promotion;

2° Improve the organisation of healthcare pathways by encouraging coordination and cooperation between health, social and medico-social care providers, in particular by mobilising the primary care teams referred to in article L. 1411-11-1 and the territorial professional health communities referred to in Article L. 1434-12;

3° Promoting access to care, prevention and support, in social, geographical and organisational terms, particularly for people with disabilities or loss of autonomy and the most disadvantaged;

4° Prepare the healthcare system for the management of exceptional health situations as set out in the ORSAN plan mentioned in article L. 3131-11.

Article R1434-6

Le schéma régional de santé comporte des objectifs visant à :

1° Développer la prévention et la promotion de la santé ;

2° Améliorer l’organisation des parcours de santé en favorisant la coordination et la coopération des acteurs de l’offre sanitaire, sociale et médico-sociale, en particulier en mobilisant les équipes de soins primaires mentionnées à l’article L. 1411-11-1 et les communautés professionnelles territoriales de santé mentionnées à l’article L. 1434-12 ;

3° Favoriser l’accès aux soins, à la prévention et à l’accompagnement, sur les plans social, géographique et de l’organisation, notamment des personnes en situation de handicap ou de perte d’autonomie et des personnes les plus démunies ;

4° Préparer le système de santé à la gestion des situations sanitaires exceptionnelles déclinées dans le dispositif ORSAN mentionné à l’article L. 3131-11.
