Article R2212-15 of the French Public Health Code

The doctor or midwife will give the woman a written document indicating the precise address and telephone number of the relevant department of the health establishment that has signed the agreement. The doctor or midwife will inform her that she may be admitted to this facility at any time.

The woman is also given a liaison sheet, drawn up jointly with the health facility that has signed the agreement, containing the relevant information from her medical file. This document is given by the woman to the doctor or midwife in the health facility when she is admitted.

Le médecin ou la sage-femme remet à la femme un document écrit dans lequel sont indiqués l’adresse précise et le numéro de téléphone du service concerné de l’établissement de santé signataire de la convention. Le médecin ou la sage-femme lui indique la possibilité d’être accueillie à tout moment par cet établissement.

Il est remis également à la femme une fiche de liaison, définie conjointement avec l’établissement de santé signataire de la convention, contenant les éléments utiles de son dossier médical. Ce document est remis par la femme au médecin ou à la sage-femme de l’établissement de santé, lors de son admission.
