Article L224-78 of the French Consumer Code

The contract includes:
1° The information mentioned in articles L. 224-73 and L. 224-74;
2° Where applicable, changes made to the same information in accordance with the provisions of article L. 224-77;
3° An indication of the identity and place of residence of the parties;
4° The date and place of its conclusion, and the signature of the parties;
5° A withdrawal form separate from the contract, conforming to a model determined by joint order of the Minister for Consumer Affairs and the Minister for Justice.
The page or pages of the contract relating to the existence of a right of withdrawal and the procedures for exercising it, as well as the prohibition on advance payment, shall be signed by the consumer.
One or more copies of the entire contract are given to the consumer when the contract is concluded.

Le contrat comprend :
1° Les informations mentionnées aux articles L. 224-73 et L. 224-74 ;
2° Le cas échéant, les modifications intervenues sur ces mêmes informations conformément aux dispositions de l’article L. 224-77 ;
3° L’indication de l’identité et du lieu de résidence des parties ;
4° La date et le lieu de sa conclusion, ainsi que la signature des parties ;
5° Un formulaire de rétractation distinct du contrat, conforme à un modèle déterminé par arrêté conjoint du ministre chargé de la consommation et du ministre de la justice.
La ou les pages du contrat relatives à l’existence d’un droit de rétractation et à ses modalités d’exercice ainsi qu’à l’interdiction de paiement d’avance sont signées par le consommateur.
Une ou plusieurs copies de l’ensemble du contrat sont remises au consommateur au moment de sa conclusion.
