Article R2223-30 of the French General Code of Local Authorities

The order form includes the agreement and signature of the person who placed the order. It contains, in addition to the information mentioned in article R. 2223-26, the following entries:

– surname and first name of the deceased;

– date of birth of the deceased;

– date of death;

– date and time of committal;

– date and time of the funeral service;

– date and time of burial or cremation ;

– surname and first name of the person who placed the order;

– address of the person who placed the order;

– relationship to the deceased of the person who placed the order;

– amount of the total sum, including all taxes.

Le bon de commande comporte l’accord et la signature de la personne qui a passé commande. Il contient, en plus des informations mentionnées à l’article R. 2223-26, les mentions suivantes :

– nom et prénom du défunt ;

– date de naissance du défunt ;

– date du décès ;

– date et heure de la mise en bière ;

– date et heure du service funéraire ;

– date et heure de l’inhumation ou de la crémation ;

– nom et prénom de la personne qui a passé commande ;

– adresse de la personne qui a passé commande ;

– lien avec le défunt de la personne qui a passé commande ;

– montant de la somme totale, toutes taxes comprises.
