Article R614-3 of the French Consumer Code

Any natural or legal person wishing to be included on the list of mediators provided for in Article L. 615-1 communique à la commission d’évaluation et de contrôle de la médiation mentionnée au même article, outre les informations mentionnées aux 3° à 9° de l’article R. 614-1 :
1° Its contact details and website address;
2° A statement of reasons justifying its appointment as consumer mediator;
3° Information on its structure and the methods of financing its activity as consumer mediator, where applicable the costs of its service billed to the professional, as well as, where there is an entity grouping together several mediators, the methods of financing this entity, the level of remuneration and the duration of the mandate of each of them;
4° A description of the internal conduct of the mediation.
The mediator shall notify the commission without delay of any change in this information.

Article R614-3

Toute personne physique ou morale qui souhaite être inscrite sur la liste des médiateurs prévue à l’article L. 615-1 communique à la commission d’évaluation et de contrôle de la médiation mentionnée au même article, outre les informations mentionnées aux 3° à 9° de l’article R. 614-1 :
1° Ses coordonnées et l’adresse de son site internet ;
2° Une déclaration de motivation justifiant sa désignation comme médiateur de la consommation ;
3° Les informations sur sa structure et les modalités de financement de son activité de médiateur de la consommation, le cas échéant les frais de sa prestation facturés au professionnel, ainsi que, lorsqu’il existe une entité regroupant plusieurs médiateurs, les modalités de financement de cette entité, le niveau de rémunération et la durée du mandat de chacun d’entre eux ;
4° Une description du déroulement interne de la médiation.
Le médiateur notifie sans délai à la commission toute modification de ces informations.
