Health, social and medico-social professionals and, where appropriate, the structures employing them, as well as prevention and occupational health services, in the exercise of their missions under article L. 4622-2 of the Labour Code, may request support for the coordination of healthcare pathways which they consider to be complex, in order to improve the service provided to the population and to contribute to the structuring of the healthcare pathways mentioned…

The support system for coordinating complex healthcare pathways: 1° Provides a comprehensive response to requests for support from professionals, including reception, analysis of the person’s situation, referral and networking, access to specialised resources, follow-up and enhanced support for situations, as well as care planning. The support systems for coordinating complex healthcare pathways have a governance structure that ensures balanced representation of players from the social, medico-social and health sectors, including in particular representatives of users, the departmental council and territorial professional health communities. This governance ensures that the principle of subsidiary intervention by the support system in relation to that of the professionals mentioned in article L. 6327-1 is respected.

Establishments authorised to provide home hospital care may participate in the operation of one or more support systems.

The local information and coordination centres mentioned in article L. 113-2 of the Code de l’action sociale et des familles (Social Action and Family Code) may join the scheme mentioned in article L. 6327-2 of this Code, by decision of the Departmental Council.

For activities subject to authorisation under article L. 6122-1 requiring special expertise, specific regional mechanisms can organise specialised support for healthcare professionals, health establishments and regional health agencies.

The conditions of application of this chapter shall be laid down by decree.
