Article R49-13 of the French Code of Criminal Procedure

The ticket and payment card mentioned in article R. 49-1, if they are sent pursuant to article 529-10, are accompanied by a request for exoneration form specifying the conditions for admissibility of the request provided for in article 529-2, the methods of payment of the deposit, as well as the penalties provided for by the articles 226-10 et 441-1 du code pénal et R. 49-19 of this code.
L’avis d’amende forfaitaire majorée adressé en application de l’article 529-10 specifies the conditions for admissibility of the claim provided for in article 530, the methods of payment of the deposit, as well as the penalties provided for by articles 226-10 and 441-1 of the Penal Code and R. 49-19 of this code.
When the request for exoneration or the complaint is accompanied by the information provided for in b of 1° of Article 529-10, it specifies the surname, first names, sex, date of birth and address of the presumed driver, as well as his driving licence number.
When the request for exoneration or the claim is accompanied by the document provided for in d of 1° of article 529-10, the latter contains the relevant information specified by joint order of the Minister of Justice, the Minister of Transport and the Minister of Road Safety, derived from data recorded by the system for recording driving delegation data.

Except in the cases provided for by articles 226-10 and 441-1 of the Criminal Code, for the author of a request for exoneration or a claim falling under the provisions of Article 529-10 of this code, to give inaccurate or erroneous information pursuant to b of 1° of this article is punishable by the fine laid down for fifth-class offences.
