I.-The therapeutic plan envisaged for each cancer patient undergoing treatment, as well as any significant changes in therapeutic orientation, including the discontinuation of cancer treatment, are the subject of a collegial discussion in a multidisciplinary consultation meeting. For adolescent and young adult patients aged between 15 and 24, the inter-regional multidisciplinary paediatric oncology consultation meeting or the multidisciplinary consultation meeting for adults guarantees the involvement of a double medical expertise in adult oncology and child and adolescent oncology, either through the participation of healthcare professionals in the multidisciplinary consultation meeting, or by obtaining additional opinions.

In order to promote patient access to therapeutic trials, the authorisation holder, if necessary with the support of the specific regional cancer system, provides the multidisciplinary consultation meetings which it organises with access to the directories of available clinical trials. In application of article R. 6123-91-10, the authorisation holder must keep a written record of the assessment of patients’ needs for supportive oncology care and, where appropriate, of the support or referral of patients to this care, in order to ensure traceability.

The authorisation holder must take the necessary steps to offer appropriate treatment to elderly cancer patients at risk or losing their autonomy. This organisation makes it possible to identify frailty in these elderly patients, and to support them, if necessary, for the purposes of a geriatric assessment and geriatric follow-up within the healthcare organisation or towards the corresponding hospital or outpatient care.

The holder organises a specific multi-annual training plan for each of the cancer treatment modalities for which it is authorised and intended for all healthcare professionals contributing to oncology treatment. This multiannual training plan is revised when there is a significant change in the cancer treatment practices, techniques or equipment used within the establishment.

The authorisation holder must guarantee the secure sharing of dematerialised documents, in particular the pathological anatomy and cytology report, the multidisciplinary consultation meeting form, the personalised care programme and the personalised post-cancer programme, with the healthcare professionals contributing to the cancer care pathway and the patient.
