Depending on the patient’s clinical condition, the authorisation holder offers each patient treatment in at least three of the following therapeutic practices: -masso-kinésithérapie; -ergothérapie; -orthophonie; -psychomotricité; -prise en charge neuropsychologique ; -adapted physical activity.
Care is organised in such a way that each patient receives at least two sequences of treatment, including at least one sequence of individualised care, depending on his or her clinical condition, on each working day in the case of full hospitalisation or on each visit in the case of part-time hospitalisation.
In addition to the premises provided for in article D. 6124-177-1, the authorised site includes an emergency room equipped to perform emergency procedures and cardiac resuscitation while awaiting transfer to a cardiac intensive care unit. The emergency room includes one or more beds equipped with cardioscopes.
The holder of the authorisation has the following facilities on site: 1° A technical exploration centre equipped with echocardiography, stress test and telemetry facilities; 2° A technical rehabilitation centre equipped with a cardiac monitoring system and various training devices; 3° An emergency trolley including a defibrillator, with access to medical fluids and vacuum, close to the rehabilitation rooms.
In addition to the teams provided for in article D. 6124-177-3, the multidisciplinary team includes: 1° At least one physiotherapist; 2° At least one dietician; 3° At least one psychologist.
The coordinating doctor specialises in cardiovascular medicine or physical and rehabilitation medicine, and in this case has proven training or experience in cardiology. In the latter case, the authorisation holder ensures that patients have access to a doctor specialising in cardiovascular medicine.
The authorisation holder offers each patient, depending on their clinical condition, treatment in at least two of the following therapeutic practices: -masso-kinésithérapie; -ergothérapie; -diététique; -prise en charge psychologique; -éducation thérapeutique ; -adapted physical activity.
Care is organised in such a way that, depending on the patient’s clinical condition, at least two individual or group treatment sequences are provided each working day in the case of full hospitalisation, or each time a patient is admitted on a part-time basis.
Medical continuity of care is ensured by a doctor specialising in cardiovascular medicine. At least one nurse is permanently present in the rehabilitation rooms alongside patients. A doctor specialising in cardiology intervenes immediately if necessary.
